🍻 You must be over the age of 21 to enter the bar, and tournament. Confirm you are 21+.

💉 We are requiring that all attendees be vaccinated against COVID-19. Please confirm you are vaccinated, or have received an exemption from SLC West Circuit TOs. Requests for exemptions will be handled by the SLC TOs at their discretion.

🤒 If you feel any COVID-like or cold/flu-like symptoms prior to your trip, we would like to ask you to stay home. If you start to feel symptoms during your trip, we would like to ask you to drop out of the tournament and do your best to quarantine for the remainder of your trip. We will help in any way we can, including with finding a replacement player for your team if possible.

🎥 I consent to being on the player camera stream and having my name and picture pulled from my Hivemind account. Any exceptions must be cleared with TOs and handled at their discretion.
